that at the time of the arrest I informed said defendant of his/her arrest, of the cause of the arrest, and of my authority to make it. Thereafter, I demanded that a police officer of the City of Corona, California, take said defendant into custody pending his/her release in accordance with California State Law.
The above defendant was arrested by me under authority of Section 834, Section 837, and Section 841 of the Penal Code of the State of California. I further state that the offense for which the defendant was arrested by me was committed in my presence.
I hearby acknowledge Section 148.5 of the Penal Code of the State of California which reads, “Every Person who reports to any peace officer listed in Section 830.1 or 830.2, District Attorney, or Deputy District Attorney that a felony or misdemeanor has been committed, knowing the report to be false, is guilty of a misdemeanor.”
